+ What are the Center's hours of operation?

Monday — Friday: 7 a.m. — 3:30 p.m.

+ Who can use the Insight Health Center?

All active teammates and their dependents (spouse/domestic partner and children ages 5+) are eligible to use the Insight Health Center


+ Can my spouse or dependents use the Insight Health Center?

You bet! Your spouse or domestic partner and/or children (ages 5+) are eligible to use the Center.


+ Do I call for an appointment or just walk in?

To make sure you can get in to see our provider in a timely fashion, we ask that you make every attempt to schedule online or call for an appointment before walking to the Insight Health Center. The Center will have limited number of same day appointments available. If you do choose to walk in without a scheduled appointment, depending on the severity of your condition, you are not guaranteed to be seen the same day


+ How can I contact the staff and make an appointment?

You can schedule an appointment at mypremisehealth.com or call 1-480-409-6100 to make an appointment. Given our current volumes and commitment to an amazing patient experience, walk-ins are discouraged unless it is an urgent matter that needs our immediate attention.


+ Is there a fee for services?

Preventive care is free; all other visits are $10. Team members and families without Insight Insurance can be seen for acute problems for a $30 access fee.


+ Will I see a doctor or a nurse for care?

You will be seen by Roxie Anderson who has over 25 years of experience in primary care.


+ Can I receive medication at the Center?

In many cases, the answer is yes. The Center has a limited supply of medications on-site primarily over the counter. For all other medication needs, your provider will write prescriptions which you can fill at the pharmacy most convenient for you.


+ Will I have to change my primary care physician because of the Insight Health Center?

As a Patient Centered Medical home that has the ability to perform full encompassing Primary care we hope you will consider us as your Primary moving forward. We believe in gaining your trust and hope you have an experience here like no other. If you are happy with your current primary that is awesome. Our team is happy to see you for short-term acute needs if you cannot get an appointment with your primary care provider.


+ Who manages the center?

Premise Health operates the center.


+ Is my health data secure and private? Will my employer have access to my medical records?

Your personal health information is confidential and your data is kept secure. Insight operates in accordance with HIPAA and works diligently to protect all health records. Premise Health can’t share any personal health information without your permission. Rest assured, your personal health information is not shared with your employer. Additionally, Premise Health operates through a private network and software systems that cannot be accessed by Insight Associates. Notice of privacy practices are available for review within the Center.
